We ask you for personal information so that you can receive appropriate care and treatment. This information is recorded on computer and we are registered under the Data Protection Act. The practice will ensure that patient confidentiality is maintained at all times by all members of the practice team. However, for the effective functioning of a multi-disciplinary team it is sometimes necessary that medical information about you is shared between members of the team. Other NHS agencies such as the Common Service Agency (Registration Dept) and GG&C Health Board require personal details in order to receive payment for services provided to you.
As we are now 'paper light', all consultations are recorded on the computer. This enables us to analyse various aspects of health care and to produce an annual practice report.
Anonymised patient information will be used at local and national level to help the Health Board and central government plan services eg for diabetic care. If you do not wish anonymous information about you to be used in such a way please let us know.
Anyone who receives information from us is also under a legal duty to keep it confidential.
